A semen analysis is the first test worth doing when a couple is struggling to conceive - and the single most informative one. Male factor is a contributing cause in around half of cases, yet it is routinely the last thing tested.
Your analysis is performed by the andrology laboratory at The Doctors Laboratory (TDL), 76 Wimpole Street. TDL is one of the few UK laboratories that analyses fresh samples produced on site, in a private room at their Patient Reception. That matters more than it sounds: motility and vitality begin to fall within an hour of production, so a sample produced at home and carried across London does not give a result you can rely on.

Your report covers the full WHO panel and is interpreted against the WHO 2021 (sixth edition) reference values:

Appearance, liquefaction, viscosity, pH and the presence of leucocytes (a marker of possible infection) are assessed alongside these.
A result below a reference limit does not mean you cannot conceive, and a result above it is not a guarantee that you can. These are the fifth centile of men who fathered a child within twelve months - not a pass mark. That is exactly what your GP consultation is for.
How it works
- Order online. £220, including your GP consultation.
- Book your online GP consultation. We email you a personal booking link as soon as you order. Your GP takes a brief history and tells you how to prepare - most importantly the abstinence window.
- Book your sample appointment. Once your consultation is complete we send you a link to choose a slot at TDL's Patient Reception on Wimpole Street. You produce the sample on site in a private room.
- Results in 2-3 working days, with your GP available to talk them through.
Preparing for the test
Abstain from ejaculation for 2 to 3 days before your appointment - no fewer than 2 days and no more than 7. Both a shorter and a longer gap change the result, which is the most common reason a sample has to be repeated. Mention any recent illness with a high temperature, and any medication or supplements, to your GP: a fever can suppress sperm production for up to three months.

Methodology
Manual and computer-assisted assessment of volume, concentration, total sperm number, motility (total and progressive), vitality and morphology, with appearance, liquefaction, viscosity, pH and leucocyte screening. Reference values follow the WHO laboratory manual for the examination and processing of human semen, sixth edition (2021).
